2008 - 2009 REGISTRATION & ENROLLMENT

Make your enrollment easier and faster by doing the following:

First, if you are a new student or a returning student that needs to update your phone numbers or address, print and fill out the above Registration Form (#1).

Second, print Payment Options Form (#2), check off the payment option you prefer and sign.

Third, print the Policies & Procedures form (#3), read and sign. Keep a copy for your records.

Fourth, if you are paying by credit or debit card for a one time payment or monthly auto debiting, please print the Autopay Form (#4), fill out and sign. Returning students with valid checking or credit or debit card’s on file with the school need only sign the form to give the school authorization for payment.

Bring in all signed forms to the studio and confirm schedule. Tada! You are now ready to dance the year away!
